    1. faulty gasket

    2. faulty/damaged/warped machined surfaces

    3. incorrect head-stud torque or tightening sequence

    4. overheated engine

    5. modified compression ? (excessive combustion pressure)

    6. the head gasket poltergeist

     

    any of the above?

    Do you know what brand of Gasket was used ?  New cylinder studs ?
